Missed Appointment:
If you are unable to keep an appointment, please notify our office immediately.
If an appointment is missed without a 24 hour notice, there will be a charge of $60.00.
Medical Records:
Processing medical records requires 7 to 10 business days to complete.
A $15.00 process fee applies to each request.
Forms/Letters:
Completion of forms and/or letters require 7 to 10 business days.
A fee will be applied to each request.
Medication Refills:
Please submit any medication refill requests by calling or texting 919-466-7540 at least 5 to 7 business days in advance to avoid interruption in your medication regimen.
Prior Authorization:
Please allow 7 to 10 business days for prior authorization requests to be processed. Some requests may take additional time, depending on your insurance provider.
